Why this book

Written for the three people who all have to say yes.

Every ICHRA decision has three stakeholders, and if any one of them isn't convinced, the initiative stalls. If you're a CFO staring at a renewal that blows up your forecast, this book gives you the financial model. If you lead HR and worry your people will revolt if you "touch the health plan," you'll find the communication playbook that got one organization to 93% satisfaction. If you're a broker, you'll see how ICHRA becomes a scalable part of your book of business instead of a threat to it.

The case study the book is built around

RFK Community Alliance's $1.4 million question

Facing yet another double-digit renewal, RFK took a risk on an ICHRA most of their leaders had never heard of. One year later: budget stabilized, employees more satisfied than under the old plan, and a savings number most CFOs would fight for. The book walks through exactly how — and where it could have gone wrong.

From the opening pages

The first thing you'll read.

In 2024, RFK Community Alliance stood at a breaking point. Their traditional group health plan was set to renew with yet another crushing double-digit increase. The options were grim: cut programs, freeze hiring, or shift more costs onto employees who were already stretched thin. Instead, they took a risk on something most of their leaders had never heard of: an Individual Coverage HRA, or ICHRA. One year later, they had saved 1.4 million dollars, stabilized their benefits budget, and given employees more choice and control over their coverage. That is the 1.4 million dollar question at the heart of this book: how did they do it, and could you?

Questions from inside the book's own FAQ

Before you request a copy.

What is an ICHRA and how does it work?
An Individual Coverage Health Reimbursement Arrangement is an employer-funded benefit that reimburses employees for individual health insurance premiums, instead of the employer buying and managing a single group plan.
What are the main advantages for employers?
Cost control instead of chasing double-digit renewals, claims risk shifting to the individual market instead of sitting with the employer, and the flexibility to set different contribution amounts by employee class.
Who is eligible to participate?
Almost any W-2 employee group, defined using permitted classes like full-time, part-time, seasonal, salaried, hourly, or geographic region. Employees also need to be enrolled in a qualifying individual plan to receive reimbursements.
What are the main steps to implement, and how long does it take?
A typical rollout follows a 90-day playbook: strategic planning in the first 30 days, plan design and legal setup in days 31–60, then communication and enrollment prep in days 61–90 before going live.
Can Medicare-eligible employees participate?
Yes — Medicare-eligible active employees are often excellent candidates, since their premiums are usually lower than ACA marketplace premiums for equivalent coverage.
